Practice Overview:

• Our new associate will join 3 other Hospice Physicians and 3 CRNP’s who enjoy the benefits of working for the area’s largest employer.
• Join a well-established hospice program started in 1987

Position Responsibilities:

• Oversight and manage patient pain and symptoms.
• Determine hospice Eligibility
• Admissions new patients.
• Triage nurse calls
• Opportunity for remote virtual visit.
• Weekends 3 patients at CC
• Weekends 1 or 2 at Muhlenberg
• Weekends outlying area: connect with RN’s by phone

Job Qualifications:  

• A network champion for Hospice Medicine and a passion for the clinical work
• Master's Degree from an Accredited CRNP program
• CRNP - Certified Registered Nurse Practitioner PA and / or other state as required. Upon Hire
• Maintain appropriate institutional, departmental, and state professional certifications and continuing education requirements.
• Preferably 2 years’ experience practicing Hospice Medicine
